DO NOT enter into FDM4. Fill out this order form , and send to your OMG CRR (If unsure who your OMG CRR is ask check the Master Rep Assignment list). If the customer needs a quote, tell you OMG CRR that you need an order approval.
Specials can be viewed by accessing Sales Deck and selecting "Special" from the document drop down.
**Reminder, the print must be exactly the same on both garments: size, location, and ink colors. You do not get an ink color change. You can change the garment color but you cannot change anything regarding the print/design.**
Includes standard decoration in ONE LOCATION:
o Screen print is up to two-colors. $2.00 more for three color logos
Decoration costs for an additional location:
o Screen print: $2.00 one color, $3.50 two color, $4.50 three color
